DESCRIPTION:

Eastern Plumas Health Care is a small, non-profit, critical access hospital district, providing comprehensive medical services to Plumas County since 1971. We operate a 9 bed acute care hospital at our main Portola campus, which includes a 24 hour, physician staffed, emergency room and ambulance service. In addition, three primary care medical clinics and a dental clinic offer residents a full spectrum of health care services in the Graeagle, Loyalton, and Portola communities. There are also hospital-based Skilled Nursing Facilities in Portola and Loyalton. Surrounded by the Sierra Nevada Mountains with beautiful lakes and trails for hiking, biking, fishing and skiing. POSITION SUMMARY:

The Resident Assessment Coordinator (R.A.C.)  of the Skilled Nursing Facility (SNF) is an RN under the direction of the SNF Manager and is in charge of all MDS completion, transmissions, planning and implementing all plans of care, and holds all patient care conferences. He/She assists the SNF Assistant Director of Nursing and Director of Nursing in all aspects of patient care. The R.A.C works closely with each disciplinary to make sure all Resident Assessments are completed and that each Plan of Care is followed.  The R.A.C.is responsible for completing and transmitting all required State and Federal Minimum Data Sets.  The R.A.C. initiates and organizes all the Interdisciplinary Care Conferences and completes and updates the Patient Care Plans as required.

EDUCATION/EXPERIENCE

 

  • Graduate of an accredited school of nursing licensed to practice as a Registered Nurse in the State of California.
  • Must have knowledge of State and Federal Regulations.
  • Must have ability to assess Skilled Nursing residents.

PRINCIPLE ACCOUNTABILITIES:

  • Work with the Charge Nurse in Assessing and Notifying the Physicians with Changes in Conditions. Acts as a backup for the Charge Nurse as needed. Assists the Charge Nurse as needed.
  • Work with Social Services, Activities, Therapies, and Nursing to complete all Resident Minimum Data Sets and Comprehensive Assessments as outlined in Medicare, Medical requirements, including Fall Assessments, Side Rail Rationales, and Mini Mental Assessments.
  • Responsible for completing & printing permanent Plan of Care for all Residents in conjunction with the MDS.  The R.A.C. is also responsible for reviewing and updating Care Plans Quarterly, Annually and as needed.
  • Initiates and organizes Resident’s Interdisciplinary Care Conferences.
  • Responsible for overseeing that the Charge Nurse’s “Weekly Nursing Summaries” are completed and correct.
  • Assists with admissions, transfers, and discharges.
  • Oversees the Activity, Social Services, and Therapy personnel to ensure that accurate documentation takes place, that the plan of care is followed, and that  the  resident’s  needs and interests are met.
  • Make sure personal belongings of residents are marked with their names.
  • Make sure all heights, weights and vital signs are done on all SNF admissions, that weights are done at least monthly and according to physician orders.
  • Attends and participates in the Resident Care Policy Committee which will meet at least yearly.  The committee is composed of the Medical Director, Administrator, SNF Manager, R.A.C., Activity Director, Dietary, Pharmacy,  Social Services.
  • Attends and participates in SNF Quarterly meeting.
  • Will oversee the Restorative program in conjunction with the RNA.
  • Work in Co-op with the billing office to assist Residents with financial issues.
  • Attends work shops and in-services appropriate to the job description. Keeps informed of new Medicare and MediCal requirements.
  • Participates in State Licensing Surveys.
  • The R.A.C. will act in the absence of the SNF Manager
