Job Description

The LCSW or LMFT, under the direction of the Clinic Director, is a self-directed individual requiring full participation and dedication within the Behavioral Health Team. The purpose of this position is to provide a variety of clinical services that include initial intake, assessments, brief therapy, crisis evaluation, phone triage with clients and referrals to appropriate services. The LCSW or LMFT has the ability to work with patients and families who are experiencing a variety of psychiatric, medical and social problems utilizing individual, group and family counseling skills by providing direct therapy/counseling to clients to help them gain insight and understanding into issues adversely affecting their lives, such as mental illness, family or marital issues, psychosocial issues and issues related to addiction and abuse. This position will support primary care physicians in managing mutual patients.

Minimum Requirements of the Position

The following education or combination of education and experience are required for the position.

· A doctorate degree in psychology from an accredited institution with a California psychology degree program approved by the American Psychological Association, or a master’s degree in social work, psychology, counseling, psychiatric nursing or appropriate related field from an accredited institution.

· Possession of license for a Licensed Clinical Social Worker (LCSW) or Licensed Marriage and Family Therapist (LMFT) issued by the California State Board of Behavioral Sciences Examiners, or as a Licensed Clinical Psychologist (CP) by the California Board of Psychology.

· Must have a valid CA driver’s license, have a good driving record and show proof of insurance coverage.

· BLS Healthcare Provider for CPR required

Preferred

· Completion of supervisory courses to supervise registered ASWs/Registered Psychologists.

· LCSW or LMFT who provides clinical supervision must possess a current and valid California license for at least two years prior to the commencement of supervision and complete a 6-hour supervision training within the two-year period immediately preceding supervision, and every renewal period when supervising according to the BBSE guidelines as required.

Principal Accountabilities Include by are not limited to:

· Provides direct outpatient care to patients age 16 and older experiencing mild to moderate anxiety and/or depression utilizing evidence based therapeutic modalities

· Works in the capacity of an independent, licensed provider in a competent, capable, professional manner which exemplifies the current standard of care and practice in therapy.

· Performs assessments on new and returning patients as required.

· Assesses the emotional, psychological and developmental status of patients utilizing a biopsychosocial approach.

· Provides assessment to diagnose and treat mental health disorders and psychosocial disorders.

· Develops and updates treatment plans to help patients meet their treatment goals following current standards of practice.

· Initiates evidence based on therapeutic treatment modalities and established treatment goals.

· Identifies appropriate therapeutic treatment approach to help patients gain insight, understanding and identify issues and behaviors which adversely affect their lives, physical and mental health.

· Participates in case conferences and team meetings to review individual needs and treatment plans.

· Communicates and coordinates care with other providers as needed.

· Communicate, when necessary, with the family and the patient in person, by letter or by telephone.

· Communicates and coordinates care with outside referral sources on the care of mutual patients.

· Provides timely and comprehensive documentation of patient assessment, treatment, problem areas, goals and progress in the Electronic Health Record (EHR).

· Maintains documentation with compliance and quality assurance requirements of the organization and department.

· Assures continuity of care for all patients in their care.

· Provides other clinical duties as designated by the Behavioral Health Program Manager or Director of Outpatient Clinics or designee.

· Additional duties as assigned.
