Eastern Plumas Health Care is a small, non-profit, critical access hospital district, providing comprehensive medical services to Plumas County since 1971. We operate a nine bed acute care hospital at our main Portola campus, which includes a 24 hour, physician staffed, emergency room and ambulance service. In addition, four primary care medical clinics and a dental clinic offer residents a full spectrum of health care services in the Graeagle, Greenville, Loyalton, and Portola communities. There are also hospital-based Skilled Nursing Facilities in Portola and Loyalton. The hospital district serves over 10,000 permanent residents living in small communities scattered throughout Plumas and Sierra Counties. 

Summary: 

This position reports to the Director Of Nursing and the role is split between both the Loyalton and Portola campuses.The Part time DSD will be approved by the state to assist the current Full Time DSD with providing in-services and orientation of new employees while the Full time DSD is giving the NATP- which is 6 weeks twice a year or more if needed. The Part time DSD can also provide in-services that have been approved when the Full time DSD is unable to, or the need arises. **This position can be combined with a Full-Time employee or it can be a separate Part time position. **

 Responsibilities 

  • Conducts a skills analysis and provides skill training to new staff.
  • Plans and delivers all staff in-services addressing mandatory topics.
  • Schedules and delivers no less than 24 hours of CNA training annually to all shifts. 
  • Prepares/obtains files and lesson plans for SNF employee training programs.
  • Maintains a current schedule of all students and outside instructors. Reviews student records for history and physical screenings. 
  • Creates and oversees daily schedules to ensure sufficient staff to enable proper coverage of resident care.
  • Maintains personnel records per Title 22, Section 72533 (a), (b) and (c), ensuring files are up to date and secured. 
  • Interview and teach CNA course 2-3 times per year. Build and revise program content. 
  • Must be willing to seek out new methods and principles and be willing to incorporate them into existing training practices.
  • Prepares and submits DHS renewal applications.

Qualifications

  • The applicant must take a state approved class within 90 days of becoming the DSD. The class is 24 hours of training.
  • 1 year of experience in long-term care facility providing direct patient care. 
  • 2 years of full-time experience as a licensed nurse. 
  • 24 hours of training in planning, implementing and evaluating educational programs, approved by the Board of Registered Nursing or an accredited educational institution. 
  • Must possess a current, unencumbered, active license to practice as a Registered Nurse (RN) or Licensed Vocational Nurse / Licensed Practicing Nurse (LVN / LPN) in CA.
  • Must have or obtain DSD certification as required per State regulations.
  • Must be able to speak, read, and write the English language.
