Eastern Plumas Health Care is a small, non-profit, critical access hospital district, providing comprehensive medical services to Plumas County since 1971. We operate a 9 bed acute care hospital at our main Portola campus, which includes a 24 hour, physician staffed, emergency room and ambulance service. In addition, four primary care medical clinics and a dental clinic offer residents a full spectrum of health care services in the Graeagle, Loyalton, and Portola communities. We are looking for a dedicated LVN to work night shifts in our Skilled Nursing Facility in Loyalton. 5k Sign on Bonus, Full Benefits, Paid Holidays

$5,000 sign on bonus that pays as follows: receive $1500 at sign on, $1500 at 90 days, $2000 at one year.

Shift: At least 2 night shifts and 1 day shift a week.

POSITION SUMMARY:

Assesses, plans, and implements the nursing care of residents within the skilled nursing setting. Responsible for ensuring continuing of care of the residents between shifts by providing direct care as well as supervising the care given by the C.N.A.s and supportive staff members. Participates in resident and family teaching while maintaining a role as resident advocate with focus on the facility’s mission.

RESPONSIBILITIES: Job duties will include but are not limited to:

LVN Duties:

  • Supervise direct patient care performed by CNAs. 
  • Administering prescribed medications.
  • Assist in assessing patients' needs. 
  • Responsible for accurate documentation in the residents' electronic medical record.
  • Performs direct patient care. 
  • Complies with local and federal guidelines for the administration of all medications and maintenance of medical records. 
  • Ensures compliance with HIPPA and other applicable laws and regulations regarding patient privacy and the maintenance of medical records. 
  • Produces quality work/assignments in a thorough, timely and accurate manner. 
  • Maintains appropriate personal attendance, accountability and work productivity standards. 
  • Understands and applies job knowledge to effectively complete all required job responsibilities.
  • Proactively maintains the skills required to perform job duties. Complies with local and federal guidelines for the administration of all medications and maintenance of medical records.

EDUCATION/EXPERIENCE:

  • The following education or combination of education and experience are required for the position:
  • Must be currently licensed in the State of California.
  • Must be CPR certified.
  • Must be able to read and communicate effectively in English.
  • Completes annual education requirements.
  • Maintain regulatory requirements.
  • Exhibits genuine interest in and appreciation of adults in later years of life.
  • Must demonstrate a maturity level that enables him/her to perform the job in a professional and organized manner.
